What should I expect on my first visit?
When you arrive to your first visit with us, you will be asked to provide the following information:
- Photo Identification
- New Patient Forms (click here to download forms)
- Physician’s Prescription/Referral
- Insurance card
- Pay any co-payment, deductible or co-insurance amount (credit, check, cash)
- Workers’ Compensation/L&I claim forms
- Authorization form from insurance company (if required)
- List of Medications with Dosages
On your first visit, you will be evaluated by one of our Occupational Therapists and treatment options will be reviewed. Your initial evaluation will last approximately 60-75 minutes.
Please arrive 15 minutes early for appointment to allow for completion of paperwork.
What should I wear?
Wear comfortable clothing that will allow us access to the area we are treating.
How do I know what my benefits for Occupational therapy are?
Though we accept most forms of insurance, you should contact your insurance by calling the number on your insurance card and ask for your “outpatient occupational therapy benefits”, verify we are a provider as well. They will give you information on your deductible, co-payment and number of visits they allow. Each patient should be aware of their benefits prior to arrive to our office. We can assist you if needed.
How much time should I give if I need to cancel or change my appointment?
We ask you to please give us at least a 24 hour notice.
